CBID Application

The following application must be completed in the student’s own hand. Please be sure to complete each required field in the application. Make sure to fill in your pastor’s email as a link to an online reference form will be emailed to your pastor. We cannot approve your application without receiving the online reference form from your pastor. Applications are considered in the order they are received. Also, please submit a $20 application fee here.

The personal information you provide in this application will be held in confidence and in compliance with the Family Education Rights and Privacy Act of 1974 (FERPA).
